Power of Attorney

My father is a resident in a dementia specific ward at an aged care home.
His deterioration was that rapid, that we had not time to arrange a power of attorney appointment.
How do we now arrange this? He could sign legal documents, however, he would not know what he is signing, let alone reasons why etc.
I really need to finalise several issues that require his authority. EG Cancellation of home phone, requesting a copy of his home insurance documentation.
