It sounds like it might be part of the Regional Assessment Service which is the assessment you have when applying for a lower level of care such as Commonwealth home Support Program, (CHSP). Could you tell us the context you heard Community Nurse Assessment used in? For example, is it for assessing someone moving into a nursing home or someone applying for Home Care? Was Community Nurse Assessment maybe used in the context of being in a remote/ rural community?
